The Challenge

- ✓Conducted a post-mortem analysis identifying seven root causes of the go-live failure: missing data validation gates, no rollback plan, single-session training, no cutover rehearsal, no hypercare staffing plan, late stakeholder sign-off, and scope creep not flagged before go-live.
